Update from the Local 124 Benefit Office:
We have 2 Final Clinic dates for the 2021 Biometric Screening. These Clinics will be back in the JATC Apprenticeship Building, Saturday September 11, 8am – 2pm, Saturday October 2, 8am - 2pm.
Members and spouses are required to complete the online Health Risk Assessment Questionnaire and the Biometric Screening to have a lower deductible of $175.00 for 2022. Please have the online questionnaire completed before attending the clinic, we will not have the computer lab available. If both are not completed by October 31, 2021, your deductible will be $525.00 for 2022.
